Do You Have To Give Out Your Social Security Number On Planned Parenthood Forms?
my best friend is 16 and needs me to take her to planned parenthood to get on the pill, hopefully for free. There are downloadable forms online and they ask for a social security number. She doesn’t know hers and she can’t ask her mother for it because she would get suspicious. Does anybody know if it is required for their services?
